Authorized Agent Request Form

As described in our Privacy Notice, residents of certain states may have privacy rights related to their personal information. If you are an authorized agent and submitting a request on behalf of a consumer, please complete this form.

Authorized agents must provide written proof of authorization to submit a request on behalf of another. We will email you directly upon receipt of your request in order to obtain a copy of the written authorization.

• Appeal: To appeal our denial of your previous privacy request
• Correction: To correct inaccurate personal information that we have about you
• Deletion: To delete personal information that we have collected about you.
